Kreative Pathways provides individualized Self-Determination Transition services to support participants and families as they enter and navigate the Self-Determination Program. We help individuals understand the program, develop person-centered goals, and coordinate services that promote independence, self-advocacy, and meaningful choice. Our approach empowers participants to direct their own supports while building skills for long-term success.

Community Integration Services

Kreative Pathways provides individualized Community Integration services that support individuals with developmental disabilities in building independence and meaningful participation in their communities. Services focus on developing social, daily living, and community navigation skills while encouraging inclusion, confidence, and self-advocacy. Supports are person-centered and tailored to each individual’s interests, goals, and abilities.

Advocacy Service

We provide advocacy support to ensure individuals’ rights are protected and their voices are heard. This includes support during Individual Program Plan (IPP) meetings, interactions with regional centers, or navigating workplace challenges. Our advocacy services empower individuals to self-advocate while ensuring access to appropriate resources and accommodations.

Life Coaching

Our life coaching program focuses on personal development and skill building. Coaches work one-on-one with participants to strengthen self-confidence, decision-making, problem-solving, and daily living skills. This service helps participants set and achieve meaningful goals, promoting independence and self-sufficiency.

Job Development & Support

Kreative Pathways provides comprehensive employment services, including career exploration, resume preparation, interview coaching, and direct job placement assistance. Once employed, we offer ongoing workplace support and job coaching to ensure long-term success, professional growth, and inclusion in the workforce.

Case Management

Our case management services provide individualized coordination of supports, ensuring participants have access to the resources they need. Case managers monitor progress, connect individuals with community programs, and maintain collaborative communication with families, service providers, and regional centers.

Entrepreneurship Support

Kreative Pathways recognizes the potential for adults with disabilities to become entrepreneurs. We provide guidance in developing business concepts, writing business plans, and building the necessary skills for self-employment. Participants receive ongoing mentorship in marketing, financial management, and networking to help turn their entrepreneurial vision into reality.​
